Community Outreach and Home Care Coordinator

JOB TYPE: Part-Time (with pathway to Full-Time)

PAY: $20–$26 per hour performance bonuses

LOCATION: Field Based (Wayne, Oakland, Macomb, and surrounding counties)

Job Summary

We are seeking a polished, driven, enthusiastic and relationship oriented professional for a hybrid role that combines community outreach and client intake. You will represent our agency in the field by building relationships with hospital discharge planners, skilled nursing facilities, and senior living communities, while also guiding families through important care decisions.

This role is ideal for someone with a clinical or care background who is confident in the field, communicates with professionalism, and builds trust quickly.

Community Outreach & Partnerships

• Build and maintain referral relationships with hospital discharge planners and case managers, skilled nursing and rehabilitation facilities, community organizations etc.
• Conduct consistent in-person outreach visits to referral partners on a weekly basis
• Present and educate partners on Nurturing Care Partners' services and referral process
• Track all outreach activity, contacts, and follow-ups in an organized and consistent manner

Client Intake & Assessments

• Conduct in-home assessments for prospective clients
• Evaluate client needs and recommend the appropriate level of service
• Clearly explain care plans, services, and pricing to families in a way that builds confidence and trust
• Guide clients through the full enrollment and onboarding process
• Coordinate internally to ensure a smooth and timely service start

Qualifications

• Prior experience in home care, healthcare, hospice, or a related field is strongly preferred
• Background as a CNA, LPN, Medical Assistant, Social Worker, or similar healthcare role is a plus
• Strong verbal communication skills and a polished, professional presence
• Comfortable operating in both professional facility settings and private client homes
• Self-motivated with the ability to manage your own schedule and work independently
• Valid Michigan driver's license and reliable personal transportation required
