Description
The RN Field Clinician position will provide nursing care which includes:
- comprehensive health and psychosocial evaluation
- monitoring of patient's condition
- health promotion and prevention
- coordination of services
- patient education
- promotion of patient/family independence
Responsibilities
- Provide or supervise skilled nursing services in accordance with physician's orders
- Asses and regularly re-evaluate the nursing needs of the patient
- Develop, initiate, implement, and update the plan of care as needed or at lease every 60 days
- Initiate preventative and rehabilitative nursing procedures as appropriate for the patient's care and safety
- Prepare clinical notes and OASIS assessments
- Coordinate services and inform physician and other personnel of changes in the patient's condition and needs
- Observe and report patient's response to treatment and any changes in his/her condition to the physician and supervising registered nurse
- Function as the patient advocate in all medical decisions affecting the patient
- Evaluate the effectiveness and outcomes of care
- Plan for the patient's discharge from home care services
Requirements
- Must hold a current license issued by the Louisiana State Board of Registered Nurse Examiner's without restrictions
- Must have a minimum of one year experience as a registered nurse
- Current CPR certification
- Critical thinking skills, compassionate
- Home Health experience is preferred but not required
- ER or ICU experience is preferred but not required
