Position: Social Worker
Location: Hazlet, NJ
Shift: Full-Time | 8:00 AM – 4:00 PM
Salary: $70,000 – $75,000

We are seeking a compassionate and dedicated Social Worker to join our team in Hazlet, NJ. This full-time role plays an essential part in enhancing our residents’ quality of life through supportive services, care planning, and collaboration with our multidisciplinary team.
Key Responsibilities:
Assist in developing, implementing, and evaluating social service programs for residents.
Maintain working knowledge of federal and state regulations, recommending policy updates as needed.
Participate in discharge planning, resident assessments, and the development of individualized social care plans.
Interview residents and families to gather psychosocial histories and determine support needs.
Coordinate transportation and connect residents with appropriate community resources.
Educate residents and families on Medicare, Medicaid, and other financial assistance programs.
Collaborate closely with medical staff and interdisciplinary teams to ensure resident needs are met.
Maintain thorough, accurate, and timely progress notes and documentation.
Support quality assurance initiatives and assist during surveys and inspections.
Participate in key committee meetings, including Infection Control, Policy Advisory, and Quality Assessment.
Ensure end-of-life care decisions (MOLST/POLST) are properly documented and honored.
Qualifications:
Minimum 1 year of experience in a social services role.
Nursing home experience required.
Strong communication, problem-solving, and interpersonal skills.
Ability to thrive in a collaborative, team-oriented environment.
- Bachelor’s or Master’s degree in Social Work (required)
