What are the responsibilities and job description for the Director of Clinical Services - Home Health/Hospice position at Nurses on Wheels, Inc?
The Hospice Director of Clinical Services position will support local clinical leaders at agencies. In association with those local clinical leaders, the Director of Clinical Services will be responsible for designing, monitoring, implementing, and evaluating process and programs to meet regulatory and accreditation requirements and to achieve the highest standard of care.
Duties and Responsibilities
· Responsible for providing a high level of leadership and guidance in the establishment of programs, policies and practices that ensure that all Home Health and Hospice agencies provide exceptional clinical care in compliance with State and Federal Regulatory requirements
· Educates and assists local clinical leaders to be survey ready at all times
· Works closely with compliance resources to ensure appropriate and effective survey preparedness
· Assists local leadership in developing and implementing processes to monitor and measure all activities related to clinical, education, and performance improvement
· Provides support regarding the direction and coordination of educational programs, orientation, new hire competency systems, annual skills competency and on-going in-service educational program development
· Monitors the organization’s Outcome Based Quality Initiative (OBQI) outcomes
· Provides feedback and best practice education to staff to improve outcomes
· Coordinates activities related to clinical record review for home health and hospice data collection
· Compiles, trends and reports quality data in the following areas: patient care, safety, risk management, infection control, outcomes, and customer satisfaction
· Data is analyzed and compared internally and externally with other sources when available
· Educates and trains nursing supervisors to ensure documentation is complete and consistent with legal requirements
· Additionally, a Director of Clinical Services may be asked to step into patient homes for observations, evaluations, and mock surveys
· Helps develop initiatives and monitor agency process and progress in areas including, patient safety, risk management, infection control programs. The Director of Clinical Services will be expected to travel when needed.

Qualifications
· Current Registered Nurse
· Minimum of five years’ Home Health and/or Hospice experience preferred
· Minimum of one year in a Clinical Management role
· Proven track record of clinical management, education and management of healthcare staff
· Outstanding leadership and managerial skills
· Knowledge of clinical policies and procedures and ability to implement
· Good organization, communication, human relations skills and reliable transportation
· Must possess the ability to lead and manage clinicians and office staff for successful patient outcomes and agency performance
