Role Summary:
We are seeking a compassionate and experienced Inpatient RN Care Manager in Mukilteo, WA to join our team in Everett, WA. This full-time position focuses on delivering professional, patient-centered care management services to at-risk individuals in an acute care setting.
What You’ll Do:
- Assess patient needs and develop individualized care plans
- Coordinate care transitions and discharge planning
- Conduct utilization reviews to ensure appropriate resource use
- Collaborate with physicians, social workers, and interdisciplinary teams
- Educate patients and families on care plans and available resources
- Maintain accurate documentation and uphold compliance standards
- Participate in quality improvement and case review initiatives
- We are seeking a compassionate and experienced Inpatient RN Care Manager in Mukilteo, WA to join our team in Everett, WA.
What We Need from You:
- Active and unrestricted Washington RN License
- Associate’s Degree in Nursing (or diploma) at time of hire
- 2 years of acute care experience in a medical-surgical or inpatient setting
- Strong communication, critical thinking, and organizational skills
- Bachelor’s Degree in Nursing (or higher) to be completed within 3 years of hire
- National certification in your area of specialty at time of hire
- A year of relevant experience in care coordination or utilization review, or formal training through TIPS or Case Management Orientation.
