Help expand access to behavioral health care across Maryland.

Nurse Practitioner On Call is seeking an experienced, results-driven Community Liaison to grow our client base and establish partnerships with long-term care facilities throughout Maryland.

Our focus is psychiatric medication management and therapy, with a goal of bringing our providers directly into partner facilities to serve residents. This position is central to that growth: you will connect with decision-makers, develop relationships, advance contract opportunities, and help turn new partnerships into active care programs.

We are hiring one person to own this critical function. The ideal candidate brings direct experience in Maryland’s long-term care market, established professional relationships, and a demonstrated ability to convert outreach into lasting partnerships.

What You Will Do

Develop facility partnerships

  • Identify and pursue opportunities with nursing homes, skilled nursing facilities, assisted living communities, and other appropriate long-term care organizations.
  • Build relationships with facility owners, administrators, directors of nursing, social workers, and other decision-makers.
  • Secure meetings and present NPOC’s medication management and therapy services.
  • Guide opportunities from initial outreach through proposals, leadership-approved agreements, and service launch.
  • Coordinate with clinical and operational leadership to align proposed services with facility needs and provider capacity.

Grow referrals and community relationships

  • Develop a consistent referral network across healthcare organizations, discharge planning teams, community agencies, and other relevant partners.
  • Conduct facility visits, community outreach, presentations, and networking activities.
  • Educate partners about NPOC’s services, referral process, and access to care.
  • Maintain relationships through reliable follow-up, responsiveness, and ongoing communication.

Own the growth process

  • Build and execute a Maryland outreach and business development plan.
  • Maintain accurate records of prospects, contacts, meetings, proposals, follow-up activities, and contract status.
  • Provide leadership with regular updates on opportunities, barriers, results, and next steps.
  • Partner with intake and clinical teams to support timely referral follow-through and successful facility onboarding.
  • Identify and address partnership concerns to support retention and continued growth.
  • Represent NPOC professionally while protecting patient confidentiality and following organizational policies.

Required Qualifications

  • At least three years of relevant experience in healthcare business development, community outreach, provider relations, or a community liaison role, including direct experience in Maryland’s long-term care market.
  • Demonstrated experience working with long-term care facilities and engaging facility decision-makers.
  • A record of generating qualified referrals, developing facility partnerships, or helping secure healthcare service agreements.
  • Existing professional relationships within Maryland’s long-term care or healthcare referral community.
  • Strong presentation, relationship-building, negotiation, and follow-through skills.
  • Ability to manage the full outreach process independently, from prospecting to partnership activation.
  • Strong organizational skills and experience tracking opportunities in a CRM or comparable system.
  • Ability to travel regularly throughout the assigned Maryland territory.

Preferred Qualifications

  • Experience developing partnerships for psychiatric services, behavioral health, medication management, or therapy.
  • Experience arranging on-site clinical services within long-term care facilities.
  • Familiarity with facility onboarding, healthcare referral workflows, and service agreement processes.
  • A degree in business, marketing, healthcare administration, social work, or a related field, or equivalent relevant experience.

What Success Looks Like

Success in this role means measurable, sustainable growth, including:

  • A qualified pipeline of facility partnership opportunities.
  • Productive meetings with decision-makers and consistent follow-through.
  • Executed facility agreements and successful launches of on-site services.
  • Growth in appropriate referrals and clients accessing care.
  • Strong partner relationships and retention.

Specific performance goals will be established with leadership.

Why Join NPOC?

This is an opportunity to take ownership of a high-impact role and work directly with leadership to expand access to medication management and therapy. Your work will help determine where we grow, which communities we reach, and how successfully we bring care into long-term care settings.

How to Apply

Submit your résumé and a brief statement describing your experience in Maryland’s long-term care market. Include an example of a facility partnership or referral initiative you helped develop, your specific contribution, and the measurable result. Please omit confidential patient or former-employer information.
