TITLE: Home Health Registered Nurse (RN) – Wound Care Required
JOB SUMMARY
The Registered Nurse (RN) reports to the Director of Nursing/Supervising Nurse and is responsible for delivering high-quality home health clinical services, with a primary focus on wound assessment, treatment, and management.
This role ensures that all care provided meets established standards of quality, safety, and compliance. The RN collaborates with interdisciplinary team members (PT, OT, ST, and physicians) to ensure coordinated, patient-centered care. The RN is also responsible for identifying patient needs through comprehensive assessments and implementing and adjusting individualized Plans of Care.
ESSENTIAL DUTIES & RESPONSIBILITIES
- Provide skilled nursing care in accordance with scope of practice, with a strong emphasis on advanced wound care management and treatment
- Perform comprehensive initial assessments and ongoing reassessments of patient conditions, including detailed wound evaluations
- Develop, implement, and revise individualized Plans of Care, ensuring timely communication with the Director of Nursing and care team
- Perform and oversee wound care treatments, including dressing changes, infection monitoring, and healing progression tracking
- Deliver services requiring specialized nursing skills, including IV therapy (if applicable) and complex care procedures
- Educate patients and families on wound care, disease processes, prevention strategies, and overall health management
- Document all clinical findings, interventions, and patient progress accurately and timely
- Communicate changes in patient condition to physicians, Director of Nursing, and interdisciplinary team members
- Participate in care coordination with other healthcare professionals to ensure holistic patient care
- Supervise, educate, and support other clinical staff as directed
- Assist with clinical staff competency assessments and training, particularly in wound care practices
- Ensure compliance with all state and federal regulations, as well as agency policies and procedures
- Participate in quality assurance and performance improvement initiatives
- Promote home health services within the healthcare community
QUALIFICATIONS & EXPERIENCE
- Current Registered Nurse (RN) license in good standing in the State of Idaho
- Minimum 2–3 years of clinical nursing experience required
- Demonstrated experience in wound care is REQUIRED (must have hands-on wound care experience in a clinical or home health setting)
- Wound Care Certification (WCC, CWCN, or equivalent) strongly preferred
- IV therapy certification preferred
- Home health or community-based care experience preferred
- Strong clinical assessment, documentation, and communication skills
- Ability to work independently and manage a flexible schedule
- Ability to travel daily, including weekends, nights, and holidays as needed
WORK ENVIRONMENT & PHYSICAL DEMANDS
Environment:
Care is provided in patients’ homes and may involve a variety of conditions and environments. This role may require responding to urgent or emergent situations. Exposure to blood, bodily fluids, and infectious materials is possible.
Personal Protective Equipment (PPE):
Use of gloves, masks, eye protection, and protective clothing as required.
Physical Requirements:
- Ability to stand, walk, bend, squat, kneel, reach, and lift
- Must be able to lift up to 50 pounds with or without assistance
- Adequate visual and hearing acuity required
