Admissions SOC RN

Overview

Join our client as an Admissions/SOC RN where you will play a vital role in providing compassionate care to patients in their homes. This position offers a structured schedule with opportunities for additional earnings through on-call duties if desired.

Key Responsibilities

• Provide admissions and start-of-care nursing services to patients in a home setting
• Coordinate care with other members of the healthcare team to ensure continuity and quality of care
• Ensure compliance with all applicable healthcare regulations and documentation standards
  
  

Schedule and Shift Details

• M-F; 10a-6p (Start and end times vary from day to day – expected to be available from 10am – 6pm daily.)
  
  

Travel

• Territory includes multiple counties in the surrounding areas.
  
  

Qualifications

Required

• Hospice (1 years)
• 1 years of experience in ANY of the following:
• Home Health Care
• Epic Home Health
• Home Health
• Management (1 years)
• High School/GED
• Valid Registered Nurse (RN) in Illinois (IL)
• ALL of the following valid licenses/certifications:
• American Heart Association Basic Life Support (AHA BLS)
• American Heart Association Advanced Cardiac Life Support (AHA ACLS)
