Full-Time, 40 Hour Work Week, Paid Bi-weekly

General Description

Under the supervision of the TCOOMMI Program Director, the Case Manager (CM) performs moderately complex (journey-level) rehabilitative planning and instructional work. Work involves identifying client's rehabilitative needs and providing the necessary instruction in teaching self-help or self-care and vocational skills to assist clients in attaining their optimum levels of independent living and vocational goals. CM Works under general supervision, with moderate latitude for the use of initiative and independent judgment. The CM will perform functions based on the TCOOMMI Program Guidelines and Processes (PGP) and contract, DHHS, JCO, and other regulatory agencies for assigned caseload. CM will provide services to mentally ill offenders who are supervised by the parole/probation departments in order to assist them in obtaining the coping skills needed to enhance good mental health, independence and autonomy, and community integration. The CM will provide case management functions such as linking and referring the offender to intra/inter service providers and assist them in gaining and coordinating access to necessary care and services and needed resources, conduct assessments to ensure proper services based on needs, and treatment planning. The CM will also provide intensive psychosocial rehabilitation planning and instructional work to the offender that involves training and education in independent living skills and medication training and support services. The CM will also provide crisis prevention and intervention as needed. The CM will be a member of the interdisciplinary team and initiate meetings as needed to discuss or staff cases. Maintain clear TCIC/NCIC background checks and driving record.

Minimum Qualifications

Minimum Qualifications: As the Local Mental Health Authority, a qualified mental health professional (QMHP-CS) with demonstrated and documented competency in the work to be performed is defined as an employee with a minimum of a bachelor's or advance degree from an accredited college or university with a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education or early childhood intervention.

One (1) year working experience in providing case management and rehabilitative services. Related work experience and education may substitute for the required work experience. Must have reliable transportation. May use personal vehicle to conduct business. Must have a valid driver's license at all times, liability insurance and a good driving record. Have clear criminal and TCIC/NCIC background check.

Preferred Qualifications: One (1) year experience working with mentally ill offenders with mental health impairments and involved in the criminal justice system. One (1) year working experience in the judicial system and with the probation and parole departments. Additional related work experience may substitute for the required work experience. Experienced with State contracts and meeting the mandates. Have treatment team experience. Bilingual in both Spanish and English.

Knowledge, Skills, and Abilities

-In the course of being employed, understands and implements trauma informed care practices.

-Knowledge in social and rehabilitative services work or in teaching and counseling persons with disabilities. 

-Knowledge of rehabilitative assessment techniques and tools, of the rehabilitative process, of statutory requirements applicable to the rehabilitative process, and of skills and adaptations necessary for employment placement of persons with disabilities.

-Ability to assess clients' functional levels; to plan an individualized program of instruction with, or on behalf of, the client, to prepare concise reports; and to communicate effectively.

-Ability to work with mentally ill offenders who are involved in the criminal justice system
