What We're Looking For
We're looking for a full time nurse case manager to provide hourly nursing and case management to patients in Waverly and surrounding areas. The Case Manager will provide skilled nursing and will review HHA's medical documentation for the patients to monitor the patient's condition. The Case Manager will conduct assessments, recertifications, and occasionally, accompany the patients to doctors appointments. The case manager should use their experience and reason to identify issues, alert the care team and physician, and guide the care team's execution of the physician's orders.
Experience Required
Each applicant should be a registered nurse with a current Ohio license. Because this is a leadership role in an independent setting, we're looking for someone with two or more years of case management experience in home health. Case management experience in other care settings is also strongly regarded. Salary range based on experience, case management background, and licensure.
Requirements:- A current, unencumbered, active license to practice as an (RN).
- Clinical assessment skills commensurate with a proven work history in home health.
- The ability to develop/initiate/revise patient's plan of care as necessary and initiate appropriate preventive and rehabilitative nursing procedures.
- The experience to coordinate patient care, report to physician and other staff members changes in patient needs, and evaluate outcomes of care.
- Monitor assigned cases to ensure compliance with DOL requirements to include the review of HHA notes, utilization management, and weekly targeted case management.
- The ability and experience to make independent decisions related to the patients home health services, when circumstances warrant such action.
- The experience to supervise and provide clinical direction to SN/HHA providing services.
- Knowledge of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to home health.
- Effective interpersonal skills and the ability to work with an interdisciplinary care team.
- Basic understanding of computer technology, including email use.
Salary : $90,000 - $100,000
