RN Position - Middleton, WI

Our community, Hattie's Bay, formerly Sage Meadows is under new ownership.

The Registered Nurse (RN) plays a critical leadership role in ensuring the highest quality of resident care and compliance with state regulations under Wisconsin DHS 83. The RN oversees the clinical and health-related functions of the community, provides direct nursing services as needed, conducts assessments, trains and supports caregiving staff, and serves as a clinical liaison between residents, families, physicians, and external care providers.

The RN ensures that all care and medication management practices align with NuCare Senior Living’s standards of excellence and regulatory expectations.

Key ResponsibilitiesResident Care and Clinical Oversight

  • Conduct initial and ongoing resident assessments, including functional capabilities, health status, and service plan development per DHS 83.35 and DHS 83.37.
  • Review and update Individual Service Plans (ISPs) as conditions change or per regulatory timelines.
  • Ensure residents receive appropriate care and follow-up based on physician orders and changes in condition.
  • Oversee medication administration systems; ensure medications are administered per physician orders and DHS 83.37 standards.
  • Accept and document verbal or telephone orders from licensed practitioners per Wis. Admin. Code N 6.03(2)(b).
  • Coordinate resident transitions of care (e.g., hospital discharge, rehab, hospice).
  • Perform wound care, injections, or delegated nursing procedures within RN scope of practice.

Staff Training and Supervision

  • Train and supervise caregiving and medication staff on health-related services, documentation, infection control, and emergency procedures.
  • Perform medication pass observations, care audits, and competency evaluations.
  • Ensure all staff follow proper procedures for medication documentation, storage, and resident privacy.
  • Support staff development and continuing education

Regulatory Compliance

  • Maintain compliance with Wis. Admin. Code DHS 83, as well as company policies.
  • Assist in preparing for and responding to DHS surveys, audits, or complaint investigations.
  • Ensure accurate and timely completion of resident documentation and health records.
  • Participate in Quality Assurance (QA) and Performance Improvement initiatives.

Communication and Collaboration

  • Serve as the primary clinical contact for residents, families, and healthcare providers.
  • Communicate effectively with physicians, pharmacists, and home health/hospice agencies to coordinate care.
  • Participate in care conferences and interdisciplinary team meetings.
  • Maintain professional and compassionate communication with residents and families.

Qualifications

  • Current Registered Nurse (RN) license in the State of Wisconsin, in good standing.
  • Prior experience in Assisted Living, Skilled Nursing, or Long-Term Care preferred.
  • Knowledge of Wisconsin DHS 83 regulations, infection control, and medication management required.
  • Strong assessment, documentation, and communication skills.
  • Demonstrated ability to train and mentor unlicensed staff.
  • Ability to prioritize, delegate, and respond effectively in emergencies.

Pay is based on experience and certifications

Job Types: Full-time, Part-time

Pay: From $75,000.00 per year

Expected hours: 25 – 40 per week
