Key Responsibilities:
- Conduct thorough assessment calls to gather relevant patient health information.
- Identify and escalate patient needs, including prescription refill requests, appointment scheduling, and connecting patients with community resources.
- Develop and implement comprehensive care plans based on patient assessments.
- Perform Behavioral Health Integration (BHI) assessments to address the mental health aspects of patient care.
- Provide patients with clear information about their health conditions, treatment plans, and self-management strategies.
- Collaborate with patients to set and achieve realistic health goals.
- Assist patients with troubleshooting medical devices and provide guidance on device setup.
- Respond promptly to patient inquiries and follow-up on calls as needed.
- Manage appointment rescheduling and regularly check and return voicemails.
- Contribute to a collaborative and positive work environment, embodying the Nsight culture of positivity.
Qualifications:
- License: An active compact LPN/LVN license is required.
- Experience: Passion for patient care, strong ability to connect with patients with empathy and compassion. A passion for patient education is essential.
- Work Environment: Quiet, HIPAA-compliant workspace.
- Skills: Moderate to advanced computer skills.
