About the Role
This is a hybrid position combining telehealth with scheduled in-person visits to Adult Family Homes, Assisted Living Facilities, and Memory Care communities throughout North Snohomish County and Skagit County. Face-to-face visits are required periodically to meet Medicare guidelines and provide the highest quality of care.
Primary service area includes:
- North Snohomish County (Arlington, Marysville, Stanwood, Lake Stevens, Granite Falls, and surrounding communities)
- Skagit County (Mount Vernon, Burlington, Sedro-Woolley, Anacortes, and surrounding communities)
Applicants should reside in North Snohomish County, Skagit County, or nearby to accommodate scheduled in-person patient visits while enjoying the flexibility of a hybrid work environment.
No private home visits. All in-person care is provided in Adult Family Homes, Assisted Living Facilities, and Memory Care communities.
Patient Population
You'll care for older adults experiencing a variety of behavioral health and psychiatric conditions, including:
- Dementia and behavioral disturbances
- Depression
- Anxiety disorders
- Bipolar disorder
- Schizophrenia
- Insomnia
- Mood disorders
- Other geriatric psychiatric conditions
Responsibilities
- Conduct comprehensive psychiatric evaluations (approximately 60 minutes)
- Provide medication management and follow-up visits (approximately 30 minutes)
- Evaluate patients experiencing acute psychiatric changes
- Develop individualized treatment plans
- Collaborate closely with primary care providers, caregivers, facility staff, and families
- Maintain accurate and timely documentation within the EMR
- Deliver compassionate, evidence-based psychiatric care focused on improving quality of life

Qualifications
Required
- Master's or Doctorate from an accredited Nurse Practitioner program
- Current Washington State ARNP license
- Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) certification
- Washington State RN license
- DEA registration
- Strong communication and collaboration skills
Preferred
- Experience caring for geriatric patients
- Experience with dementia-related behavioral management
- Long-term care, assisted living, memory care, or skilled nursing experience
- Telehealth experience
Compensation
$130,000–$165,000 annually (Full-Time)
Compensation is competitive and based on experience, qualifications, and schedule.
Part-time opportunities are also available.
