Speech-Language Pathologist – PRN (Home Health)
Nova Health First (Nova Home Care) | Indianapolis Metro Area | Per Diem / PRN
Nova Health First is hiring a Speech-Language Pathologist (SLP) on a PRN basis to evaluate and treat home-bound patients with dysphagia, cognitive-communication, and speech-language disorders across the Indianapolis metro.
What you'll do
- Conduct SLP evaluations, treatment sessions, and discharges in the home
- Complete OASIS-E assessments when assigned as the case-opening clinician
- Treat dysphagia, aphasia, dysarthria, cognitive-communication deficits, and voice disorders
- Provide caregiver education and train on compensatory strategies
- Coordinate with PT, OT, SN, MSW, and referring physicians
- Document in WellSky in a timely manner
Requirements
- Active Indiana SLP license in good standing
- ASHA CCC-SLP (CFY candidates considered with supervision plan)
- Master's degree in Speech-Language Pathology
- Can pass a federal or national expanded background check
- Current BLS/CPR certification
- Valid driver's license, reliable vehicle, and auto insurance
- Home health experience preferred; dysphagia management skills essential
