Responsibilities
The RN Navigator will conduct outreach, recruitment, and retention efforts for HS participants. The RN Navigator will provide family-centered, strengths-based case management/care coordination (CM/CC) to facilitate access to medical care and community-based resources through screening and uptake through multiple modalities, including in-home, HS site, and/or virtually, to meet the unique needs of participants including high-touch support when indicated. The RN Navigator will document enrollments, group-based education, referrals and linkages to additional services; complete standardized HS data collection tools at appropriate times of perinatal phases as recommended by HRSA through REDCap®. The RN Navigator will create a service care plan to meet each participant’s/family’s unique needs and provide ongoing monitoring and support. This may include individualized (CM/CC), referrals, translation/interpretation, transportation, eligibility assistance, environmental health risk reduction, health literacy, outreach, and a discharge plan. The RN Navigator will assist participants in uptake of healthy behaviors before, during, and after pregnancy through health promotion and education.
Qualifications
Required:
- Three years of experience in labor/delivery or maternal child health
- Bachelor Degree (Nursing)
- Registered Nurse (IN)
