Job Requirements
- Bachelor (or higher degree) of Science in Occupational Therapy from an accredited program.
- Successful completion of National Certification Examination for OTR.
- Current state licensure or license eligible.
- Assumes responsibility for ongoing continuing education and professional development.

Job Relationships
Responsible to and reports to the Director of Therapy/Rehab.
Job Responsibilities
- Provide and direct occupational therapy services to patients including assessment, treatment, program planning and implementation, related documentation, and communication under physician’s orders.
- Provide occupational therapy services to patients and initiate referrals when appropriate and in accordance with Northwoods Rehab Policies and Procedures.
- Conduct screenings of patients at regular intervals to determine need for intervention/treatment.
- Evaluate patients to obtain data necessary for treatment planning and implementation; conduct specialized evaluations as indicated.
- Interpret and communicate evaluation findings to patients, family, and other team members.
- Establish individual patient schedules in conjunction with other team members.
- Develop treatment plans by identifying a target list, including short and long-term goals and methods to achieve identified goals.
- Coordinate treatment plan with patient, family, and other team members.
- Implement and conduct treatment as outlined in treatment plan.
- Monitor patient’s response to intervention and modify treatment as indicated to attain goals in accordance with physician orders.
- Develop appropriate home or community programming to maintain and enhance patient performance in their own environment.
- Document results of patient’s assessment, treatment, follow-up, and termination of services.
- Adapt treatment to meet the individual needs of the patient.
- Provide, recommend, fabricate adaptive devices or other equipment; train patient, family, and appropriate staff in the use or application of orthotics and prosthetics.
- Participate in discharge planning.
- Supervise and direct COTAs in the implementation of the treatment plan.
- Supervise Rehabilitation Techs/Aides, students, and volunteers.
- Review the quality and appropriateness of total services delivered and individual occupational therapy programs for effectiveness and efficiency, using predetermined criteria.
- Participate in patient, family, and staff education.
- Confer formally and informally with other team members in coordinating the total rehabilitation program of the patient.
- Adhere to Northwoods Therapy procedures regarding documentation of occupational therapy services.
- Attend in-service meetings and facility meetings as required.
- Provide in-service education to members of the patient care team and education to the community.
- Provide treatment in accordance with established standards of practice and department procedures.
- Maintain safe and clean work area and adhere to facility safety standards, complying with all Infection Control, Universal Precautions, and OSHA standards for healthcare professionals.
- Consistently demonstrate sound judgment in evaluation, planning, implementation, and follow-up of patient therapy programs.
- Maintain positive relationships and rapport with coworkers, supervisors, patients, family members, facility personnel, and administration.
- Report to work on time and coordinate schedule for efficient patient treatments.
- Assist with and participate in Quality Assurance Program as required.
- Adhere to established confidentiality standards.
- Project a positive and professional image at all times.
- May be asked to travel to participate in home assessments within a reasonable distance from the facility.
- Maintain passwords for required software and keep all passwords secure.
- Perform other duties as assigned and as required to effectively perform the responsibilities of the position and in the best interest of Northwoods Rehab.
Salary : $40 - $50
