What are the responsibilities and job description for the MDS Coordinator position at Northwood Skilled Nursing and Rehabilitation?
1. Coordinates and monitors the facility’s Interdisciplinary Team for MDS scheduling, tracking, and plan of care meeting.
Responsible for managing and coordinating the facility’s MDS information system.
Monitors the facility interdisciplinary participation, assisting in the provision of related educational and training needs.
Assists the facility with the preadmission/admissions processes.
Monitors facility compliance with the RAI Process.
Responsible for timely generation of the assessment schedule, the completion and submission of MDS assessments, CAA’s, and Triggers; responsible for timely completion of nursing plans of care; monitors MDS Department for same; coordinates interdisciplinary plans of care for timely completion and compliance.
Assures that the resident, resident sponsor, and the attending physician, are included in the development of the resident plan of care; facilitates education as it relates to the RAI Process.
Oversees the Interdisciplinary Team Meetings to ensure that they are conducted regularly to update the plan of care, including discharge planning, establish short and long-term goals, evaluate the care plan and the implementation of the plan for effectiveness.
Oversees the communication with all interdisciplinary team members including all staff, attending physicians, residents, residents’ sponsors, and resident families and facilitates follow-through
Maintains communication with the MDS software contact and communicates to the facility
Participates in continuous quality improvement activities that monitor the quality and effectiveness of the services, ongoing
Coordinates with Rehabilitative Services at the facility and oversees rehabilitation utilization.
Performs regular chart reviews to ensure validated documentation with regard to the MDS, CAA’s, and
care plans.
Weekly review of facility Medicare rate, Case mix and monthly quality indicators, and completes
comparisons to identify declines, educational needs, etc. and develops plans for improvement.
Coordinates the facility case management and utilization review processes.
Ensures all requested data and information are communicated to corporate representatives in a
timely manner.
Serves as a facility resource person in regard to the RAI process.
Generates facility reports as requested by facility.
Participates in facility meetings, committees, etc. as direct by facility
