Position Title: Service Coordinator- YIP
Reports to: Project Manager, YIP
Compensation: $55,000 annual Robust Benefits Package

The Chicago Department of Family and Support Services (DFDD) Youth Services Division, in partnership with the Chicago Police Department have created the Youth Intervention Pathways program. Northwestern Settlement is seeking a full time service coordinator to implement this program.
Key Duties/Responsibilities
- Case manage up to 30 youth (ages 10-17) referred through Chicago Police Department
- Administer risk assessment tool, conduct intakes with youth and family, create a case plan, connect with referrals, follow-up and case closures
- Maintain communication and connection with youth through a variety of methods to increase youth engagement
- Work with referred youth to identify strengths, needs, and a pathway to support and services
- Utilizing a strengths-based approach, work with youth to create plans that can be accomplished within 90 days
- Maintain records in Cityspan and track/monitor youth outcomes
- Communicate with referral partner within 7-10 business days
- Inform referral partners of any changes to youth circumstances that may impact service delivery
- Performs other duties and functions as assigned by the VP, Youth and Young Adult Services or Program Manager-YIP
Education/Experience
- Bachelors/Masters in Social Work or related field
- Experience working with youth ages 10
- Experience working with multiple partners
- Case management experience preferred
- Due to the nature of this role, both written and oral fluency in Spanish is highly preferred

Compensation
