What are the responsibilities and job description for the Adult Acute Case Manager position at Northwestern Counseling & Support Services Inc?
Northwestern Counseling & Support Services is seeking a compassionate and dedicated individual to join our Acute Case Management team as part of our greater Adult Home and Community-Based programming. This case management position serves adults diagnosed with severe mental illness with high acuity needs. We are seeking someone who is person-centered and passionate about working the population of individuals who intersect with criminal justice, legal, and substance use systems. This position is essential in supporting individuals to lead more independent and fulfilling lives.
Key Responsibilities:
- Providing services across various community-based settings, focusing on supportive counseling, symptom management, daily living activities, social and interpersonal skills development, money management, and advocacy.
- Ability to build rapport with individuals who may be difficult to engage or resistant to treatment.
- Maintain a positive attitude, work well within a team, and stay organized to manage caseloads effectively.
- Collaboration and care coordination with community resources and agencies, as well as with internal or external service providers.
- Develop and oversee treatment plans, service coordination, crisis intervention, and representative payee services,
- This position includes night and weekend flexibility for medication deliveries as needed.
Qualifications:
- Reliable transportation with the capacity to transport clients.
- Experience in mental health case management is a plus, but we are willing to train the right candidate.
