What are the responsibilities and job description for the Acute Care Case Manager position at NORTHWESTERN COUNSELING & SUPPORT SERVICES INC?
Northwestern Counseling & Support Services is seeking an individual to join our Acute Care Case Management Team. This team is connected to our greater Adult Services Case Management Team that supports individuals aged 18 & up with diagnoses of severe and persistent mental illness. Individuals served within this team often present with the highest acuity needs, including discharges from inpatient psychiatric admission and high-risk behaviors. Individuals served within this position may be heavily involved with the Department of Corrections, other law enforcement entities and, substance use systems. This role is ideal for candidates with a highly flexible, compassionate, and person-centered approach to service delivery.
Key Responsibilities:
- Providing services primarily in community-based settings.
- Focusing heavily on crisis intervention and symptom management.
- Ability to build rapport with individuals who are resistant to treatment.
- Collaboration with inter-agency providers and emergency services team.
- Coordination with external service providers.
- Flexibility for client medication deliveries.
Qualifications:
- Reliable transportation with the capacity to transport clients.
- Bachelor’s degree is preferred, but an associate degree in human services or relevant experience will be considered.
- Experience in mental health case management or similar high acuity experience.
