What are the responsibilities and job description for the Intellectual Disability position at Northwestern Community Services Board?
The Intellectual Disability (ID) Case Manager serves to assist in the provision of services to intellectually disabled individuals of the jurisdiction(s) served by his/her clinic site position location. Primary duties include individual assessment, service plan development, residential placement and follow-up, information and referral, case recording, and liaison with other agencies and facilities which serve intellectually disabled individuals.

QUALIFICATIONS:
- A combination of mental health work experience or relevant education, and training, which indicates the possession of knowledge, skills, and abilities necessary to perform essential job duties required.
- At least one year of experience working in ID services to meet Department of Medical Assistance (DMAS) requirements required.
- Must possess a valid driver's license, a good driving record, and the ability to provide your own transportation.
ADDITIONAL REQUIREMENTS:
- May be exposed to allergenic environments from client's homes, including dust, smoke, pet contamination, cleaning supplies, etc
- May be exposed to clients with infectious diseases.
- Must be able to drive for long periods of time to travel in and out of the catchment area as needed.
- Must be able to walk distances, climb and descend stairs, maneuver in and out of agency vehicles.
- Prolonged periods sitting at a desk and working on a computer; and
- Must be able to lift/push/pull up to 25 pounds at times
SALARY: Starting at $52,788
