The Developmental Disabilities (DD) Supervising Case Manager serves to assist in the provision of services to Intellectually Disabled (ID) and DD clients of the jurisdiction(s) served by his/her clinic site position location. Primary duties include organizational supervision, client assessment, service plan development, residential placement and follow-up, information and referral, case recording, and liaison with other agencies and facilities which serve ID/DD clients.

MAJOR DUTIES AND RESPONSIBILITIES:
Assesses client’s comprehensive needs for services and supports through interviewing client and/or family and utilizes appropriate assessment tools.
Develops individualized written case management services plans which outline service needs.
Links clients to services and supports specified by the service plans.
Maintains complete and accurate client records, in accordance with all applicable regulations.
Coordinates, monitors, and advocates for client’s receipt of required services from various providers.
Provides supportive intervention and follow-up with clients and families.
Performs pre-discharge service planning and coordination with state and private residential facilities.
Serves as a resource on ID/DD services to other Community Services Board personnel and community agencies.
Develops and submits required statistical data for reporting purposes.
Provides guidance to case managers pertaining to Agency policy, licensure, Medicaid, and the Department of Behavioral Health and Developmental Services (DBHDS) regulations.
Assesses individuals for inclusion on the statewide DD waiting list.
Provides consultation for Emergency Services staff and providers when needed.
Attends and participates in required training through the Department of Behavioral Health and Developmental Services (DBHDS) and the Agency. Participates in clinic staffing, as needed.
Approves leave requests, timesheets, and travel forms of assigned case managers.
Assists in the recruitment and performance management process for Case Managers in collaboration with the DD Services Supervisor.
Approves services completed in Credible for assigned case managers.
Assists with training and development of assigned case managers.
Provides resource and case consultation to clinic case management staffing.
Performs other duties as assigned.

QUALIFICATIONS:
A combination of work experience or relevant education, and training, which indicates the possession of knowledge, skills, and abilities necessary to perform essential job duties required.
Bachelor’s degree earned from an accredited college or university in a relevant human services field required.
At least five years of experience working in DD services required.
Must possess a valid driver’s license, a good driving record, and the ability to provide your own transportation.

ADDITIONAL REQUIREMENTS:
May be exposed to allergenic environments in client’s homes, including extreme heat, humidity, dust, smoke, pet contamination, cleaning supplies, etc.
May be exposed to clients with infectious diseases.
Must be able to drive for long periods of time to visit clients in and out of the catchment area as needed.
Must be willing and able to assist clients with basic duties such as shopping, carrying personal items and putting household items away.
Must be able to walk distances, climb and descend stairs, maneuver in and out of agency vehicles.
Prolonged periods sitting at a desk and working on a computer.
Must be able to lift/push/pull up to 25 pounds at times.

SALARY: $56,549 – $60,577
