Position: Case Manager (RN)- BH
Position Summary: A registered nurse with strong leadership abilities, communication, and problem solving skills who, with minimum supervision, collaborates with all members of the treatment team in assessment of patient needs related to discharge planning and coordinates interventions with other members of the treatment team to meet those needs in a quality cost effective manner. The case manager conducts utilization management and insurance reviews as indicated by managed care companies; assists physicians with Medicare certification and recertification requirements. Collaborates with social worker/discharge planner to ensure that legal process requirements are met for patients on involuntary status. Supports an organizational culture for Service Excellence and practices the Service Excellence and practices the Service Excellence Standards to all customer groups
Job Duties/Responsibilities: Demonstrates leadership in the provision of departmental services through a collaborative process designed to integrate services/functions throughout NWTHS. Assesses or coordinates the assessment of patient healthcare and education needs and collaborates with all members of the healthcare team to meet those needs. Orchestrates patient hospital process for provision of seamless, efficient care. Demonstrates the ability to effectively allocate human and material resources to meet patient care needs. Coordinates utilization management and insurance reviews as indicated. Complies with annual education requirements and safety and injury prevention standards to minimize risk to self and co-workers.

Qualifications:Education, Training, Experience and Licenses/Registrations Required:
- Registered Nurse currently licensed in the state of Texas with a Bachelor’s Degree (in nursing or other healthcare related field) or equivalent experience and clinical expertise, and a minimum of five years of clinical experience.
- Excels in interpersonal and communication skills.
- Current BLS certification from the American Heart Association upon hire or within 30 days of start date and maintain certification throughout employment. This must be obtained prior to providing patient care.
