Adult Day Program Assistant (DSP)

Description

General Duties:

The DSP is responsible for direct supervision of services to the clients of the PURSUIT Adult Day Program including support and assistance in all areas addressed in the Individual Personal Plans of the client.

Essential Functions:

• Participate as part of the Interdisciplinary Team providing input concerning evaluation, objectives, progress, and revision for client's goals (ISP).
• Provide skills development and maintenance to clients in accordance with the Individual Personal Plan and record all necessary data.
• Provide, supervise, and participate in scheduled social and recreational activities for the clients.
• Participate in monthly staff meetings for advanced training and to discuss issues/needs directly related to providing supports and services to the clients.
• Implement approved behavior plans for the clients.
• Complete NWSRA driver's training and drive agency vehicles (14 passenger bus) for program activities.
• Work with DSP Team to prepare and implement meaningful activities; (i.e. set up supplies and equipment, inspect new sites for potential hazards, adapting activities as needed, completing required record keeping).
• Maintain client/family confidentiality and conduct themselves professionally.
• Adhere to all policies and procedures established for Clearbrook & NWSRA.
• Complete mandatory training annually.
• Perform any other duties and responsibilities as assigned.

To be qualified for this position an individual must meet the following standards and be able to perform the essential functions of the job:

• Must pass post-employment physical and drug test within 30 days
• Must pass an Illinois state criminal background check with no disqualifying conditions
• Must pass a TB test within 30 days of employment
• Must have a valid driver's license, with acceptable driving record
• Must be DSP certified within 120 days, Training is provided at no charge. DSP Certification not required for hiring eligibility. 
• Must pass literacy test at an eighth grade level

Environmental Conditions:

• Work may be performed in a variety of settings with varying levels of cleanliness, noise, etc.
• Occasional exposure to blood borne or airborne pathogens
• Occasional exposure to combative clients
• Heavy lifting required
• Hazards typically present in a home environment
