Nurse Navigator - Hoffman Estates Clinical Staff

Role and Responsibilities

• RN staff nurse responsibilities and works in concert with staff to complete treatment in offices during clinic hours.
• Documents all phone messages and interaction with patients as needed, either via triage software or text note.
• Provide education and is a resource for staff nurses regarding new treatments being used by their MD or informs the Clinical Nurse Educator regarding new treatments.
• Coordinates care for patients needing home health, palliative or hospice care, and any treatment at an outside facility
• Completes paperwork for prescriptions, FMLA, disability and other forms as needed.
• Reviews upcoming schedules with MD and addresses unscheduled in EMR.
• Assists MD with reviewing patient test results.
• Maintains OCN certification
• Comply with the OSHA regulations
• Participate in the NWOH safety program
• Attend all mandatory trainings & complete quizzes as assigned.
• Report hazards
• Suggest controls
• Report occupational illnesses and injuries

PHYSICAL DEMANDS AND WORK ENVIRONMENT:

• occasionally required to lift up to 25 pounds
• Specific vision abilities required for this job include: close vision, distance vision, peripheral, depth and ability to adjust or focus.
• exposure to fumes, airborne particles or toxic chemicals and hazardous materials.
• Frequent exposure to bloodborne and airborne pathogens or infectious materials
• protective clothing or equipment is required that include:  gloves, closed-toe shoes, protective eyewear and gowns.
• the noise level in the work environment is moderate.
• perfume/cologne and scented lotions or make-up are not allowed in clinical space.
• dress-code required – see manager.

Additional Notes

Attends all mandatory meetings unless otherwise approved by manager.  It is the employees’ responsibility to get notes from any missed staff meeting. Represents NWOH in a professional and positive way when interacting with peers, patients, and other professional ancillary services.

Qualifications:

• Minimum 1 year oncology experience at nwoh. (Exceptions will be considered)
• Current IL state License in good standing
• vaccinations up to date

EDUCATIONAL REQUIREMENTS:

• Registered Nurse from an accredited school of nursing.
• Chemotherapy & Biotherapy Certification from the Oncology Nursing Society
• BSN preferred, not required.
• Current CPR
• OCN within two years of accepting position
