We are seeking a dedicated Family Nurse Practitioner (FNP) or Physician Assistant (PA) to join our Shelby primary care team in beautiful Oceana County.
About the Role
The Medical Provider delivers comprehensive ambulatory primary care services across the lifespan as part of our patient-centered healthcare model. Working collaboratively with physicians, nurses, and support staff, this role ensures high-quality, coordinated care for all patients, including underserved and medically vulnerable populations.

Key Responsibilities
Provide primary care services within scope of practice
Assess, diagnose, and manage acute and chronic health conditions
Develop patient-centered treatment plans and provide education
Collaborate with physicians and care team members to ensure continuity of care
Accurately document and code patient encounters in the electronic health record
Participate in peer review, quality improvement, and clinical team meetings
Work collaboratively with interpreters and community partners
Support NMHSI’s Medical Home Model and organizational initiatives
Qualifications
For FNP:
Licensed Registered Nurse in Michigan
Master’s degree from an accredited Nurse Practitioner program
Certified Family Nurse Practitioner (FNP)
Active NP specialty certification in Michigan
For PA:
Graduate of an accredited Physician Assistant program
NCCPA certified
Active Michigan PA license
For both:
Active DEA license (or ability to obtain)
One or more years of family practice experience preferred
Commitment to serving diverse and underserved populations
Bilingual English/Spanish preferred, but not required
