Location: Northwest Human Services – West Salem Clinic, 1233 Edgewater St. NW, Salem OR 97304
Job Status: Full-time, Monday-Friday, 8am – 5pm
Sign-on Bonus: $3000.00 (Internal candidates that are currently employed here at NWHS are not eligible for sign-on bonus.)

YOUR ROLE:
As our Nurse Visit RN, you’ll work side-by-side with providers, behavioral health professionals, medical assistants, and care coordinators to empower patients through education, access, and coordinated services that improve health outcomes.
SPECIFIC DUTIES:
- Conduct independent nurse visits for services such as immunizations, medication education, wound care, diabetes coaching, contraceptive injections, ABI testing, SLUMS assessments, and more.
- Collaborates with care team to ensure seamless and patient-centered care.
- Provide personalized patient education to promote self-management of chronic conditions, proper use of medications and devices.
- Ability to accurately observe, analyze and interpret general and technical data both verbal and written, and to accurately communicate this information.
- Ability to recognizes abnormal and critical clinical findings and takes appropriate action including notifying provider as appropriate.
- Current Oregon RN license.
- Minimum of 1-2 years of clinical nursing experience, preferably in an ambulatory care, community health, or FQHC setting.
- Has strong clinical assessment, problem-solving, and decision-making skills with sound clinical judgement.
- Able to provide compassionate, culturally competent care delivery.
- Current Basic Life Support for Healthcare Providers (BLS) training certificate consistent with AHA Guidelines for CPR/ECC, and covers breathing and cardiac emergencies - including CPR, AED, and obstructed airway - for adult, child, and infant patients.
- Bilingual preferred but not required.
