We are currently seeking an experienced Home Health Registered Nurse (RN) to provide skilled nursing visits in the Roscoe, IL area and surrounding portions of Boone and Winnebago Counties.

Schedule
Part-Time / PRN
Monday–Friday, Daytime Hours
Flexible scheduling based on your availability and patient needs.
Registered Nurse (RN) Responsibilities
- Manage and coordinate a patient caseload
- Complete Start of Care (SOC) OASIS assessments
- Perform ongoing skilled nursing visits and patient evaluations
- Collaborate with physicians and interdisciplinary healthcare team members
- Develop, implement, and update individualized care plans
- Educate patients, caregivers, and family members regarding disease management and treatment plans
- Monitor patient progress and communicate changes in condition to physicians and clinical leadership
- Document care accurately in Axxess EMR
- Provide wound care and other skilled nursing interventions as needed
- Ensure compliance with Medicare and agency standards of care
Qualifications
- Current Illinois Registered Nurse (RN) license
- Minimum 1 year of Home Health experience (required)
- Minimum 1 year of OASIS experience (required)
- Wound care experience preferred
- Current CPR certification
- Valid driver's license and reliable transportation with auto insurance
- Strong clinical assessment, communication, and organizational skills
- Passion for providing compassionate, patient-centered care
