What are the responsibilities and job description for the Case Manager - Health Related Social Needs position at Northwest Family Services?

This position helps participants access resources that will support housing stability, food security as well as devices to mitigate the impact of disabilities and climate change.

This is a temporary position. We need someone to work Monday – Friday, 40 hours a week from 11/24/25 – 04/24/26.

What’ll you do:

· Screen referrals and assess client eligibility based on the program guidelines.

· Obtain documentation and complete the intake forms for eligible participants.

· Provide trauma-informed, strengths-based case management.

· Collaborate with clients to develop an action plan in order to address additional needs and goals.

· Submit funding requests and then follow up to ensure transactions have been completed.

· Provide participant referrals based on availability and individual need.

· Serve as a liaison and advocate for program participants in order to mediate any issues.

· Create and maintain accurate and up-to-date case files.

· Follow up as needed

General Responsibilities

· Drive to meet community partners, clients, and vendors as needed.

· Follow the protocol, policies, and procedures of the department and agency.

· Complete and submit reports and documentation as needed.

· Maintain confidentiality of client information and comply with HIPAA guidelines.

· Provide culturally responsive services that are equitable, respectful, and relevant to people from a wide variety of cultures, backgrounds and orientations.

· Attend meetings and trainings as required.

· Regularly participate in professional development.

Who you are:

· Must have a minimum of a high school diploma or equivalent.

· Have a minimum of 2 years of experience working with vulnerable communities and people from diverse backgrounds.

· Experience working in a case management capacity with homeless or at-risk households preferred.

Supplemental Information

Prior to being hired, a background check including criminal record history will be conducted. Information from the background check will not necessarily preclude employment.

This position requires the use of a personal vehicle and a minimum level of insurance coverage for that vehicle.

· Personal liability insurance with $50,000 for bodily injury per person, with an aggregate of $100,000 per accident, or

· $100,000 combined single limit (bodily injury and property damage combined)

Job Type: Temporary

Pay: $25.00 - $28.00 per hour

Expected hours: 40 per week
