Complex Care Manager/Registered Nurse (CCM)
DESCRIPTION
Northwest Community Health Center’s (NWCHC) is looking to hire a Complex Care Manager/Registered Nurse (CCM) to support its primary care team. The CCM will work directly with vulnerable and complex patients and will oversee the development and implementation of comprehensive patient-centered care plans. He/she will be providing comprehensive care management, care coordination, medication reconciliation, health promotion, transitional care support, and perform nurse assessment monitoring care plan effectiveness and adjusting interventions as needed. Health Education will be a key role of the CCM.
The CCM will also support the implementation and operationalization of NWCHC’s medication assisted treatment (MAT) in collaboration with NWCHC medical and behavioral health providers through tracking and coordination, community outreach, patient assessment, reporting, compliance, and other activities in support of the service.
The successful candidate will be a graduate from an accredited school of nursing and hold a MT Registered Nurse license. 1-2 years experience of relevant clinical experience in healthcare is preferred. Must be able to demonstrate clinical competency, excellent verbal and written communication skills, set priorities, and work as a team player. Salary will be determined on credentials, experience and qualifications.
