Job Summary
Coordinates and implement activities of the Social Rehabilitative Program. Provide direct behavioral health treatment and community-based care services to clients on a daily basis. Evaluates and coordinate recreational, vocational, and educational services for program participants.
Essential Duties
- Coordinate and facilitate skill-building activities for Social Rehabilitative clients.
- Submit individual client progress notes daily.
- Plan, implement, evaluate, and monitor treatment activities and group sessions.
- Educate clients on Daily Living Skills to enhance their independence.
- Deliver direct clinical services and community-based care, including assessments, treatment planning, peer support, and social recreational activities.
- Collect and record data on client behaviors and progress.
- Maintain a safe, supportive, and therapeutic environment for clients.
- Supervise client interactions during group activities and encourage positive peer relationships.
- Complete all required documentation accurately and promptly within the agency’s Electronic Health Record (EHR) system.
- Obtain the State-required Behavioral Health Technician (BHT) certification within one (1) year of employment.
