QUALIFICATIONS

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required.
1.Has or willing to obtain Alzheimer’s Certificate

2.Must be a graduate of an accredited school with a BSW, recreational therapist or equivalent.

3.Must have working knowledge of Alzheimer’s and Dementia.

4.Must be qualified to assume the position by training and /or previous experience of a supervisory status, preferably in a health care and long term care facility.

5.Must possess the ability to make independent decisions and mature judgments.

6.Must have the ability and the willingness to supervise and work harmoniously with professional and non-professional personnel.

7.Must be able to read, write, and speak English in an understandable manner.

8.Must be physically and mentally capable of performing routine job duties.

9.Excellence in all aspects of quality dementia care including exceptional person-centered care.

10.Good managerial, organizational, planning, and interpersonal skills.

11.Leadership qualities that motivate and direct the memory care staff.

12.Be personable with residents, families, and staff in a professional and cooperative manner.

13.Must have compassion, tolerance, and understanding for the elderly.

14.Thorough knowledge of CMS regulations.


ESSENTIAL FUNCTIONS
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
a.Support the facility's philosophy of care and strive to achieve its goalsand objectives.
b.Be sensitive to residents' families and respond in an appropriateprofessional way as the situation requires.
c.Assign all staff to residents-spelling out break times and all specialduties to be completed.
d.Supervise all staff in performing their duties by checking their workclosely to ascertain that assignments have been completed.
e.Make daily rounds on the wing to ensure that individual Care Plans arebeing followed and assess each resident's status in accord with his/herCare Plan.
f.Assist in updating the Care Plan every 30-90 days depending on theresident's condition and level of care at the Care Plan meeting.
g.Recommend changes needed in staff and kind of work performed.
h.Participate in staff meetings in order to keep staff performance atoptimum level.
i.Make rounds with physicians and record visits in chart.
j.Be responsible for well-being of all residents assigned to his/her unitwhile on duty. Contact families regarding significant decline in resident'sstatus or transfer to hospital.
k.Ensure that nursing personnel provide treatments, diets, rehabilitativenursing care, activities of daily living, protective services, and training inself-care as stated in the care plan to ascertain that optimum comfortand care is provided.
l.Make rounds and observe individual residents who are experiencingepisodes of acute illness, deterioration in health status, recent injury,recovering from surgery, etc., so as to be acquainted with the resident'sstatus, both physically and emotionally and to ascertain that the staff isrendering proper care.
m.Make transportation and other arrangements for patients that have tosee their physician or other outside professional medical provider.
n.At all times abide by policies of the facility and ascertain that employeesunder his/her supervision do the same.
o.Prepare incident and accident reports and forward them to the ADON.
p.Ensure that a copy of the medical records or an abstract of them areforwarded with the transfer of residents to a hospital or other health carefacility.
q.Detect and correct situations that have a high probability of causingaccidents or injuries to residents and/or staff.
r.Assure that work/assignment areas are clean and free of hazardousconditions, i.e. spills, excess carts, etc. and that equipment, tools,supplies, etc. are properly stored while working, as well as before leavingsuch areas for breaks, mealtimes, and end of the work day.
s.Assure that established infection control and universal precautionpractices are maintained when performing care.

t.Follow established safety precautions when performing tasks and whenusing equipment and supplies.
u.Assure that equipment is cleaned and prepared for the next shift.
v.Report all equipment malfunctions and breakdowns to the ED as soon aspossible and keep the ED or Central Supply informed of supply needsand equipment needing replacement.
w.Assure that work schedules are followed.
x.Report all hazardous conditions or equipment to the ED andMaintenance Supervisor.
y.Maintain the confidentiality of resident information and honor his/herpersonal and property rights.
z.Attend nursing department, Care Plan, and other meetings as directed.
aa. Attend and participate in in-service educational classes and on-the-jobtraining programs as scheduled or as directed.
bb. Perform other related duties as directed by the Executive Director.
cc. Social Services designation preferred.
