Job Summary

We are seeking a Licensed Practical Nurse to join our team. In this role, you will be caring for Hospice patients in their homes. You are a Licensed Practical Nurse (LPN) with great attention to detail and a big heart. You take pride in caring for patients following personalized care plans for patients. You are able to assess, observe and communicate effectively regarding your patients care.

Responsibilities

• Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans
• Administer medications and monitor patients for side effects or reactions
• Document any changes in patient conditions and report on changes
• Maintain accurate records of patient care and discuss observations with the supervisor
  
  
  

Qualifications

• Currently licensed Licensed Practical Nurse (LPN) in state of Ohio
• Previous experience as a Licensed Practical Nurse is preferred
• Hospice experience preferred but not required
• Commitment to excellent care and customer service
• Excellent interpersonal and client care skills
• Valid driver’s license
• First aid/CPR certification required
