• Position: Patient Navigator

• Location: 1200 S York Rd. Elmhurst, IL

• Full Time 36 hours

• Hours: Monday-Friday 7:30am-4pm

• Required Travel: no

What you will do:

• Greet and welcome patients and their families, offering assistance and guidance.

• Provide a warm and compassionate presence, addressing patient questions and concerns with empathy.

• Act as a point of contact for patients and their families, ensuring they feel comfortable and supported throughout their healthcare journey.

• Assist patients in scheduling appointments with healthcare providers, specialists, and other necessary services.

• Coordinate appointment reminders and follow-up as needed.

• Communicate appointment details and instructions clearly to patients.

• Provide information on available hospital services, community resources, and support programs.

• Assist patients in accessing financial assistance programs and insurance information.

• Collaborate with social workers and other healthcare professionals to connect patients with appropriate resources.

• Maintain accurate and up-to-date records of patient interactions, services provided, and referrals made.

• Input data into electronic health records and hospital databases as required.

• Advocate for patients' rights and preferences within the healthcare system.

• Assist in resolving patient concerns and issues in a timely and effective manner.

• Collaborate with the Patient Navigation Coordinator to address complex patient needs.

• Work closely with the healthcare team, including nurses, social workers, and case managers, to ensure seamless patient care.

• Attend team meetings and contribute to patient care planning discussions.

What you will need:

• Associates Degree Health Administration Required Or

• Bachelors Degree Health Administration Required
