- Review medical histories and medications to develop individualized care plans.
- Coordinate with patients, families, providers, and the interdisciplinary team to ensure seamless care transitions.
- Explain hospice services and provide compassionate support to families during end-of-life care.
- Accurately document assessments, communications, and changes in patient condition.
- Help initiate and facilitate a smooth start of hospice services.
- Current Florida RN license and CPR/BLS certification.
- Experience in hospice or home health care preferred.
- Strong clinical skills, teamwork, integrity, and a compassionate approach to patient care.
