Seeking a compassionate Physical Therapist to deliver high-quality home health services. The role involves evaluating patients, developing personalized treatment plans, and providing skilled therapy to enhance mobility, strength, and independence.
Key Duties
- Assess patients and create individualized treatment plans
- Deliver therapy services following the Plan of Care
- Monitor progress and adjust treatments as needed
- Accurately document evaluations, treatments, and outcomes
- Educate patients and caregivers on exercises, mobility, and fall prevention
- Collaborate with physicians and care teams
- Ensure compliance with relevant regulations and standards
- Active Florida PT license
- Preferred minimum of 1 year home health experience
- Strong assessment, documentation, and communication skills
- Ability to work independently with a flexible schedule
- Reliable transportation and valid driver’s license
- Experience with electronic medical records (EMR)
- Knowledge of Medicare and home health documentation standards
- Bilingual in English and Spanish is a plus
