Role: Licensed Practical Nurse (LPN)

Responsibilities

  • Provide in-home nursing care for seniors and individuals with disabilities
  • Perform skilled nursing services such as assessments, medication administration, wound care, and chronic disease monitoring
  • Collaborate with RNs and an interdisciplinary team to follow care plans
  • Educate clients and families on health management
  • Accurately document care provided and ensure safety and compliance

Qualifications & Skills

  • Valid LPN license
  • Compassionate and patient-centered approach
  • Strong communication and teamwork skills
  • Ability to work independently in home settings
  • Attention to detail and adherence to safety protocols

Key Duties: Deliver quality nursing care, collaborate with healthcare team, educate clients and families, and maintain accurate documentation to promote health and safety in a home environment.
