What are the responsibilities and job description for the Home Care Nurse position at Northland PACE?
What if your nursing career allowed you to slow down, build meaningful relationships, and truly get to know the people you care for?
At Northland PACE, we believe older adults deserve the opportunity to live with dignity, independence, safety, and choice. Our Home Care Nurses play an important role in making that possible by providing compassionate, skilled nursing care where our participants feel most comfortable—their own homes.
We're looking for a Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our team in either a full-time or part-time capacity.
If you're a nurse who enjoys variety, values independence, and wants your work to have a lasting impact, you may be exactly who we're looking for!
As a Home Care Nurse, you'll provide hands-on, person-centered nursing care while working closely with our Interdisciplinary Team.
Your days may include:
- Providing skilled nursing care in participants' homes and at the PACE day center
- Assessing participant needs and communicating changes in condition to the appropriate providers and team members
- Responding to changes in health status, illness, or urgent care needs
- Administering medications and treatments as ordered
- Educating participants and caregivers about medications, chronic conditions, wellness, and care needs
- Supporting participants in maintaining their health and independence at home
- Maintaining accurate and timely electronic medical record documentation
- Collaborating with nurses, providers, social workers, therapists, and other members of the care team
- Adapting to changing participant needs with professionalism, compassion, and a positive attitude
- Participating in an on-call rotation as needed
We know great nurses have choices, so we work hard to create an environment where you can build a rewarding career without giving up your life outside of work.
You'll enjoy:
- Full-time or part-time flexibility
- Meaningful work with a frail elderly population
- Work-life balance with consistent scheduling
- Competitive compensation
- Comprehensive benefits for eligible employees, including health, dental, vision, retirement, PTO, and more
- Supportive, collaborative team environment
- Autonomy in your day while still having the support of an experienced interdisciplinary team
- Ongoing training and professional development
- The opportunity to develop real relationships with the participants you serve
We're looking for a nurse who is compassionate, dependable, adaptable, and confident working independently in the community.
You'll need:
- Associate or Bachelor's degree in Nursing (ASN/ADN or BSN)
- Current unencumbered North Dakota LPN or RN license
- Current CPR/BLS certification
- Valid driver's license, good driving record, and required automobile insurance
- Strong communication, organization, and problem-solving skills
- Ability to work independently while collaborating effectively with an interdisciplinary team
- Comfort traveling to participant homes and providing care in a variety of environments and weather conditions
- A genuine passion for serving older adults
At Northland PACE, you're not simply treating a patient—you're caring for a person.
You'll have the opportunity to learn what matters most to each participant, understand their goals, and help them remain safely in their homes and connected to their communities.
Our nurses are an important part of a team that believes healthcare should be personal, compassionate, and focused on the whole person.
If you're ready for a nursing position where your clinical skills matter, your voice is valued, and the relationships you build make a difference, we'd love to meet you!
Make a difference. Build relationships. Help older adults live safely and independently at home.
