What are the responsibilities and job description for the Community Health Worker, Canton, NY position at Northern Regional Center for Independent Living, Inc.?
The Community Health Worker will work with primary care providers and patients to build self-management skills and assists with accessing health care resources. The person will build relationships in the community to assist people with disabilities to maximize their independence and build better relationships with healthcare and other systems that affect the lives of people with disabilities. Direct lived experience with disability is preferred. People with any disability are strongly encouraged to apply.
NRCIL will pay for completion of CHW training which will be expected from successful candidate
Help people access community services specific to their needs
Link people to needed health care information and services
Build trust and promoting encouragement are two of their key objectives
Prioritize a sense of community connectedness
Assist people to access community resources
Build individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy
Provide peer support when appropriate
Represent NRCIL at forums and public meetings in the community
Actively promote disability rights and independent living philosophy through all activities conducted within and on behalf of NRCIL
Complete all reporting and record keeping as required.
Work outside of regular office hours when necessary.

Pay: $16.50 - $18.50 per hour
