The Case Manager supports the department by providing case management to the different units within the facility. Manages the client’s cases with a solid background of clinical knowledge and expertise and may provide some therapeutic services to clients. Responsible for the overall discharge and aftercare planning needs and placement of the client. Maintains routine contact with referral sources and providers while maintaining resource file. Participates in quality care monitoring for the department.

What You’ll Do
In this role, you’ll play a key part in supporting clients throughout their recovery journey by coordinating care, delivering services, and collaborating with a multidisciplinary team.
Core Responsibilities:
- Develop, implement, and monitor individualized service plans (ISP) based on client needs
- Provide therapeutic services and support aligned with treatment goals
- Collaborate with clients, families, and care teams to ensure effective treatment planning
- Educate clients and families on treatment options and care plans
- Maintain accurate, timely clinical documentation (assessments, progress notes, discharge planning)
- Coordinate care with internal teams, medical providers, and community resources
- Ensure all services and referrals outlined in the treatment plan are completed and followed up on
- Communicate key clinical updates to the treatment team and external partners as needed
- Begin discharge planning at admission and support smooth transitions of care
- Develop aftercare plans and connect clients to ongoing support services
- Assist clients with accessing benefits and resources (e.g., insurance, SSI/SSD, etc.)
- Provide crisis intervention when needed and support clients through acute situations
- High School Diploma or equivalent required; Bachelor’s degree in Social Work, Counseling, or related field preferred
- 1 year of experience in behavioral health, case management, or related setting preferred
- Experience working with similar patient populations is a plus
- Strong communication, organization, and teamwork skills
- CPR and de-escalation certification (or willingness to obtain upon hire)
- First Aid certification may be required based on location
