Job Overview
We are seeking a dedicated and skilled Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong background in patient care, including experience with various clinical settings such as long-term care, assisted living, and acute care environments. This role offers an opportunity to make a meaningful difference in the lives of our residents by delivering comprehensive nursing services with professionalism and compassion.
Duties
- Conduct thorough patient assessments, including vital signs, physical examinations, and health histories, to develop individualized care plans.
- Administer medications, injections, IV infusions, and treatments following established protocols while ensuring patient safety.
- Monitor patients’ conditions closely, recognizing signs of deterioration or complications such as acute pain or infectious diseases.
- Provide wound care, catheterization, tube feeding, and other specialized procedures with attention to aseptic technique and infection control standards.
- Document all nursing activities accurately using Electronic Health Record (EHR) systems and ensure compliance with HIPAA confidentiality policies.
- Collaborate with interdisciplinary teams—including physicians, therapists, and social workers—to coordinate discharge planning and ongoing care needs.
- Educate patients and their families on health management strategies, including dementia care, Alzheimer's support, pain management, and medication administration.
Requirements
- Valid RN or LPN license in good standing; additional certifications such as Basic Life Support (BLS) or Advanced Cardiovascular Life Support (ACLS) are preferred.
- Demonstrated experience in diverse clinical environments such as hospital medicine, long-term care facilities, or assisted living communities.
- Proficiency with EMR/EHR systems for accurate medical documentation.
- Experience working with geriatric patients, individuals with developmental disabilities, dementia or memory care needs is highly desirable.
- Strong understanding of infection control practices, medication administration protocols, and long-term care regulations.
- Excellent communication skills with the ability to work effectively within a multidisciplinary team while maintaining compassionate patient interactions.
