ESSENTIAL POSITION FUNCTIONS:
- Must successfully complete Working with People class and demonstrate the ability to embrace and practice the philosophies and tools of gentle teaching.
- Demonstrates support needed to promote integration into the community for the individual that adheres to “person-centered planning” and principles of self-determination.
- Locate, obtain, coordinate and collaborate with resources/services outside and inside the NLCMH system as indicated by the individual treatment plan.
- Assist or provide arrangements to access resources and events, including provision of transportation.
- Work effectively with other service providers, families, guardians and case managers concerning assessments, services implementation and consumer participation.
- Plan and implement an individualized activity calendar for each consumer based on consumer desires and interests with consumer involvement and input from family and support team.
- Ability to prepare progress notes, reports and required documentation and submit on a timely basis.
- When caring for a person you will be expected to exert physical exertion to help a person meet their personal care needs.
SUPPLEMENTAL FUNCTIONS:
- May represent NLCMHA on internal/external committees or work groups to enhance organizational performance and development and quality of care, including Quality Improvement activities.
- May participate in community education/community caregiver activities.
- May receive and assist in resolving complaints or inquiries related to services provided by NLCMHA.
- May provide daily living tasks as written in the IPOS. Task could include but not limited to, transferring bathing and toileting.
- Responsibilities as identified in position of Case Management Assistant, Behavioral Health Home Aide and Personal Emergency Response System worker.
- Review assigned enrollees Plans of Care to:
- Assist with providing community resources and supports for linking enrollees to that resource
- Obtain enrollees information and documentation necessary for care coordination
- Provide support services to BHH Team members to assess enrollee needs and a plan of care
- Assist with appointment making and coordinating transportation for enrollees
Community Support Worker may also provide any additional support needed by the BHH Team members.
QUALIFICATIONS & COMPETENCIES: NA
MINIMUM EDUCATION & EXPERIENCE:
High school degree/GED required and one year of experience is required. Training in mental health services or related field desired.
KNOWLEDGE, SKILLS AND ABILITIES:
- Basic knowledge of the Mental Health Code and the structure, guidelines and services of NLCMHA.
- Ability to communicate, understand and carry out oral and written directions effectively and clearly present information both orally and in writing on a level suitable for public groups.
- Basic computer skills.
- Learn community resources and identify procedures required to use a wide range of services.
- Ability to maintain effective working relationships and productively serve as a member of a team with employees and the public; ability to deal with problems courteously and tactfully.
- Skill in advocating on behalf of consumers, families/guardians and NLCMHA with the community at large.
- Must have a working telephone, possess and maintain a valid Michigan Driver’s License, have a good driving record, automobile insurance coverage and have access to an automobile.
- Must be willing to transport agency consumers, including in own personal vehicle.
Salary : $17 - $19
