ACT IDDT Paraprofessional

ACT/IDDT PARAPROFESSIONAL | Location to be determined: Grayling or Houghton Lake

As a member of the Assertive Community Treatment (ACT)/Integrated Dual Disorder (IDDT) Team, the ACT/IDDT Paraprofessional provides rehabilitative, skills training, support, and recovery-oriented services to individuals enrolled in the ACT/IDDT program. Services are delivered in community settings and are designed to assist individuals with serious mental illness and co-occurring conditions in achieving their recovery goals, increasing independence, improving functioning, and maintaining community tenure.

ESSENTIAL FUNCTIONS:

• Provide annual and periodic psychosocial assessments according to program and agency guidelines.
• Facilitate consumer driven treatment plans with individualized interventions and specific, measurable, achievable, relevant, and time-bound objectives.
• Provide psychoeducation, skill-building interventions, and recovery-focused supports based on stage of change and treatment plan goals.
• Assist consumers in developing and enhancing skills necessary for independent community living.
• Actively participate in the development of interdisciplinary plan of service as part of the clinical treatment team.
• Assist consumers in accessing community resources, benefits, housing, employment and educational opportunities.
• Monitor, report, and document changes in member-functioning and wellness.
• Support members through coaching, modeling, prompting, and therapeutic interventions within scope.
• Participate in daily team meetings and case reviews.
• Collaborate with community partners and ACT/IDDT team members.
• Represent the program in the community relative to the needs of the chronically ill and their families.
• Provide crisis intervention by participating in 24-hour on-call coverage on a rotating basis.
• Engage in professional development activities to enhance individual and team knowledge of mental illness, co-occurring diagnoses, treatment, and intervention strategies to assist consumers.
• Maintain timely and Medicaid-compliant documentation.
• Promote member rights, self-determination, and person-centered planning.
• Participate in training, supervision, and quality improvement activities.
• Deliver medications to recipients as needed.
• Provide appropriate transportation for service needs and/or assist the consumer in arranging transportation to achieve treatment plan goals.

QUALIFICATIONS:

• Bachelor’s Degree in Education, Human Services, Psychology, Sociology, Rehabilitation, Special Education, or related field
• Licensed or certified in as:
  • Educator
  • Occupational Therapist
  • Physical Therapist
  • Behavioral Analyst
  • Speech-Language Pathologist
  • Audiologist
  • If not licensed or certified in one of the above, Human Services Professional with a Bachelor’s Degree
• Ability to be credentialed by NLCMHA to provide services in this professional discipline.
• Ability to work effectively within a multidisciplinary team.
• Possess a basic knowledge of ACT/IDDT programs and principles acquired through MDHHS-approved ACT and IDDT-specific trainings within six (6) months of hire to an ACT/IDDT team and shall complete at least one MDHHS-approved ACT and IDDT-specific training annually thereafter.

PAY: $53,270 - $60,270

WORKING CONDITIONS/PHYSICAL REQUIREMENTS:

• This position may require irregular hours.
• This position may require travel by personal vehicle if an agency vehicle is not available.
