We are seeking a compassionate and motivated Social Worker to join our team! The Social Worker plays a vital role in supporting residents and their families by addressing social, emotional, and psychological needs. This position ensures that residents' rights are respected, care plans are person-centered, and discharge planning is coordinated effectively.
Qualifications:
- Education: Bachelor’s Degree in Social Work, human resource field, including, but not limited to Sociology, special education, rehabilitative counseling, and psychology.
- Licensure/Certification: Must meet state requirements for licensure or certification as a Social Worker.
- Experience: Minimum of 1 year of supervised social work experienmce working directly with residents in a healthcare setting
- Skills:
Strong interpersonal and communication skills
Empathy, patience, and cultural competence
Familiarity with state and federal regulations affecting long-term care
Ability to work collaboratively in a team environment
Key Responsibilities:
- Conduct psychosocial assessments upon admission and as needed to identify resident needs.
- Develop and implement individualized care plans in collaboration with the interdisciplinary team.
- Provide supportive counseling to residents and families to assist with emotional adjustment to facility placement, medical diagnoses, and end-of-life issues.
- Coordinate discharge planning, including arranging for home care, outpatient services, or alternative placements.
- Advocate for residents' rights and ensure their psychosocial well-being.
- Assist residents and families in accessing community resources such as Medicaid, Medicare, long-term care insurance, and other services.
- Participate in care conferences, departmental meetings, and quality improvement initiatives.
- Maintain accurate, timely documentation in compliance with state and federal regulations.
- Respond to and help resolve grievances or concerns from residents and families.
- May require occasional evening or weekend hours to meet resident needs or attend family meetings.
