Northeastern Vermont Regional Hospital (NVRH) is looking for a Community Health Grant and Program Manager to join the team! As the Community Health Grant and Program Manager, you will collaborate with a highly qualified team, community partners, regulators, and governmental agencies to implement population health and healthcare reform efforts that aim to enhance patient experience of care and support complex care coordination to achieve improved health outcomes. The Community Health Grant and Program Manager oversees implementation of the Vermont Blueprint for Health model in the St. Johnsbury Health Service Area, ensuring alignment with regional participation in the Accountable Care Organization (ACO), OneCareVT. The Manager will continue the long-standing commitment to collaboration across health and human service providers to successfully align and achieve population health goals in the service area.

What you’ll do day-to-day:

  • Facilitate administrative, financial, and reporting functions, such as managing regional contracts, ensuring compliance with reporting requirements for Blueprint for Health and other grants, including timely submissions of invoices and data reports.
  • Work collaboratively with the Community Health Improvement (CHI) department and NVRH staff to develop and implement programmatic strategic priorities.
  • Collaborate with the NVRH grant development team to pursue other funding opportunities that support CHI strategic objectives.
  • Develop and maintain regional and statewide relationships to ensure collaborative approaches for population health programming.
  • Work closely with the Blueprint-funded Quality Improvement Facilitator who supports practices and communities to achieve improvement of clinical quality measures, patient experience measures, care coordination, practice transformation, and reducing variation in outcomes, connected to quality improvement efforts and priorities at the community and state levels.
  • Participate and facilitate regional work groups and collaboratives to ensure safe, effective, efficient, patient-centered, timely and equitable access to healthcare and other services across the social determinants of health.
  • Develop methods to evaluate outcomes of internal and external health promotion efforts.
  • Serve as liaison to communicate changes to state-wide healthcare reform policies.

Qualified candidates should demonstrate a comprehensive knowledge of care coordination models and methods, including financial payment models for primary care and continuum of care providers, as well as process and systems improvement models. To be successful in this role, candidates should have significant experience in a healthcare environment, community engagement skills and experience utilizing data-informed approaches to achieve desired population health objectives.
