What are the responsibilities and job description for the Behavioral Health Case Manager position at Northeastern Tribal Health System?
Position Summary: Provide client centered management assistance, services, and resources to Behavioral Health Department and Patients.
Position responsibilities:
- Provides resource information to patient population for Behavioral Health.
- Establish rapport and professional relationships with area agencies and patients.
- Provide direct linkage services to patients with area resources.
- Collaborate with providers, healthcare professionals, social service agencies and support staff.
- Keep timely and accurate service documentation.
- Makes calls, emails, and faxes as needed for the Behavioral Health Department.
- Advocate on behalf of patients in need to outside agencies.
- Assess patients’ needs and support systems.
- Planning for crises and helping clients develop coping mechanisms.
- Complete Behavioral Health Department related tasks as deemed appropriate by the Behavioral Health Supervisor.
- Other duties as assigned.
Essential skills and experience:
- Bachelor’s Degree in Social Work, Psychology, Sociology, Counseling, or related field.
- Two years of experience with a mental health agency and case management.
- Excellent interpersonal and communication skills.
- Problem analysis and problem resolution at both a strategic and functional level.
