What are the responsibilities and job description for the Case Facilitator II DCS position at Northeastern Center Inc?
The DCS Case Facilitator operates under supervision of the Child and Adolescent Associate
Director. As a member of a multidisciplinary treatment team, primary job duties include support
and coordination for families in crisis as a result of DCS involvement through comprehensive case
management and community resource connections. This role focuses on enhancing family
stability and well-being by employing evidence-based practices. The ideal candidate will possess
strong interpersonal skills and a deep commitment to fostering resilience and self-sufficiency in
families.
Education: Minimum Bachelor’s degree from an accredited university preferred.
Complete state training within 18 months of hire.
Experience: A minimum of 2 or more years of one or combination of the following experience
- Clinical
- Case Management
- Skill Building
- Child Welfare
- Juvenile Justice
- Education in a K-12 school setting
- License: Must possess a valid state issued driver's license.
- First shift, full time, M-F, some scheduled appointments that may roll into 2nd shift hours.
