Spoke Care Navigator
Join NEW Alliance Behavioral Health as part of an innovative partnership with Providence Northeast Washington Medical Group (PNEWMG Colville) to expand access to Medication Assisted Treatment (MAT) for individuals affected by opioid use disorder. Through the Washington State Health Care Authority’s Hub and Spoke Model of Care, you’ll play a key role in connecting people to life-saving treatment and recovery supports.
As a Spoke Care Navigator, you will be the vital link between clients, healthcare providers, and community resources. Working closely with the Providence “Hub” and the NEWABH “Spoke,” you’ll coordinate care, support MAT patients in recovery, and ensure smooth transitions across behavioral health, medical, and social service systems.
Key Responsibilities
  • Identify and engage individuals who may benefit from MAT and other substance use disorder services.
  • Conduct screenings, assessments, and education related to MAT and recovery supports.
  • Coordinate care between clients, prescribers, pharmacies, and the Providence Hub team.
  • Monitor patient progress and support retention in both behavioral health and medical treatment.
  • Link clients to housing, employment, primary care, and social support resources.
  • Maintain accurate documentation and reporting for state and grant compliance.
  • Conduct outreach to community agencies to increase awareness of the Hub and Spoke program.
Qualifications
  • High school diploma or equivalent and two years of experience in behavioral health or related field (or equivalent education/experience).
  • Preferred credentials: SUDP, Behavioral Health Case Manager (BA/BS/BSW), LPN/RN, or Certified Peer Counselor.
  • Eligible for Agency Affiliated Counselor certification in Washington State (required).
  • Excellent communication, organization, and documentation skills.
  • Proficiency with electronic health records and basic computer systems.
  • Valid driver’s license, insurance, and reliable transportation (travel required throughout Stevens and Ferry Counties).
