The Patient Navigator – Enhance Care Management (ECM) Program is a community-based professional who serves as a guide to help individuals access and navigate the health care system, as well as related social and financial services, to improve health outcomes. The Patient Navigator performs a variety of health education, clerical, and medical support tasks both in the clinic and in the community. A primary responsibility of this role is to establish and maintain communication between patients, their families, providers, and the health care system to support coordinated care.


Reports to: Program Manager I – Enhanced Care Management (ECMP) and Director of Quality of Quality Health Equity and Innovation



Qualifications:


Patient Navigator I: 


1.    High school diploma or GED.
2.    Previous experience in a health care setting is preferred. 

Additional Requirements for all levels: 


3.    Must maintain a current Cardio-Pulmonary Resuscitation certification, in compliance with corporate CPR Policy.
4.    Good written and verbal communication skills.
5.    Ability to be trained to perform a variety of tasks.
6.    Direct knowledge of the local community.
7.    Strong interest in the community and its people
8.    Committed to ongoing learning and the ability to model the behavior and activities that support individual development and community capacity-building.
9.    Effective communication and other interpersonal skills that can ensure an ability to work with local people and organizations.
10.  Ability to effectively assist patients across various age groups, including infants, adults, and individuals with disabilities.
11.  Fluency in English (speaks, read and writes). 
12.  Fluency in Spanish (speaks, interpret, read, and write) and will be required to complete the mandatory Medical Interpretation course offered at NEVHC. 
13.  Ability to maintain absolute confidentiality about health care and other patient/client information. 
14.  Ability to maintain a professional demeanor at all times. 
15.  If working in a school based site, must be willing to submit fingerprints in a manner authorized by the California Department of Justice, with no adverse findings.
16.  Current California driver’s license, appropriate insurance coverage, and a driving record acceptable to the NEVHC’s insurance carrier (if required to drive on the job).
