Concord Orthopaedics is seeking a compassionate, organized, and patient-focused Clinical Support Nurse to play a vital role in orthopedic patient care. This position is heavily focused on post-operative care management, patient education, care coordination, and clinical support, helping patients achieve the best possible outcomes before and after surgery.

What You'll Do
As our Clinical Support Nurse, you'll serve as a trusted clinical resource for patients and staff by:
- Coordinating comprehensive pre-operative and post-operative patient care
- Performing post-operative care checks and supporting patients throughout their recovery
- Providing patient education on surgical procedures, recovery expectations, wound care, medications, and home care plans
- Assisting patients with care management needs, referrals, and discharge planning to ensure a smooth transition from surgery to recovery
- Serving as a clinical resource for our Patient Care Coordinator team by responding to patient questions through the patient portal, phone, and email
- Providing wound care expertise while educating and supporting clinical staff
- Coordinating durable medical equipment (DME) distribution, including braces, slings, and other orthopedic devices
- Developing relationships with rehabilitation facilities and monitoring patient progress following surgery
- Supporting pre-operative optimization initiatives, including nutrition, weight management, and home readiness
- Scheduling appropriate referrals and follow-up appointments
- Leading quality improvement initiatives and coordinating quality committee meetings
- Managing MIPS quality reporting and collaborating with department leaders to meet annual performance goals
- Collecting and reporting key Workers' Compensation metrics
- Leading specialty workgroups focused on standardizing patient care and improving outcomes
- Participating in professional organizations, including AORN, to identify best practices and enhance orthopedic patient care
- Performing other duties as assigned
What We're Looking For
Required Qualifications
- Graduate of an accredited School of Nursing
- Current Registered Nurse (RN) license in New Hampshire
- Current BLS Certification
- Excellent communication, customer service, telephone, and computer skills
- Ability to thrive in a fast-paced, collaborative healthcare environment
Preferred Qualifications
- Care management experience
- Post-operative surgical nursing experience
- Orthopedic nursing experience
- Experience coordinating care across multiple providers and healthcare settings
- Passion for patient education and improving the overall patient experience

Physical Requirements
This position requires frequent walking, standing, reaching, bending, kneeling, and squatting, along with moderate keyboarding, clerical work, direct patient care, and moderate to heavy lifting.
